Report Changes to your Ontario Health Card Information

All individuals are responsible for providing accurate, up-to-date vaccination information.

Residents of Niagara region who have recently updated their health card information with Service Ontario can report these updates to Niagara Region Public Health to ensure any changes are accurately reflected in the provincial COVaxON vaccination database.

Reasons for submitting this form include:

• Change in first, middle or last name
• Change of address
• New residents to Ontario who have just received an Ontario health card number
